Immunizations:
New York State law and district guidelines require that immunizations must be completed or your child will be excluded from school. Records must have a doctor’s signature/stamp to be valid.
School Physicals:
New York State requires all new students and students entering 9th and 11th grade, to have a physical examination. A valid physical is also necessary for any student wishing to obtain Working Papers and/or to participate in school sports. Any student requiring a physical exam who has not submitted a completed physical form within 30 days from the start of school will be scheduled to have their physical by the school physician. Physical forms are available in the health office and on the District Website @ www.southcountry.org .
Medications:
New York State and district guidelines require a doctor’s order and a signed permission slip for the dispensing or self-carrying of prescription and over-the-counter medication (Tylenol, Motrin, antacids, etc.) in school. The school nurse will NOT dispense any medication to students unless he/she has a doctor’s written order on file giving specific instructions to do so. Orders must be renewed each school year. Medication Forms are available in the health office upon request. Medications that will be kept in school must be brought in and picked up by a parent/guardian.
Physical Education Excuses
Any serious injury or illness that restricts a student from physical education requires a doctor’s note. The note must state the reason as well as the duration of time that the student will be excused. In addition, a clearance note is required in order for your child to resume physical education/sports activity. If the injury requires the use of crutches, elevator or mini bus services, a note from the doctor is required. All doctor’s notes are to be brought into the health office.
